Due Week 1

Submit your proposed Service Learning Project for approval: Service Learning Project Selection Form (Word Fill-in) . Provide a brief description of planned product/service learning services. Please include:

• What the planned product/service is
• How you will develop and implement the project
• Who the target audience is
• What is the expected outcome of the project

Due Week 2

Review the  health care systems articles, conduct your own research, and submit a two page typewritten report describing financing mechanisms that support public and community health.
Note:  Please include references.

Due Week 3

Complete and submit Community Public Health (Word Fill-in). Describe how the 10 essentials of Public Health relate to the patients you are caring for in this course.

Due Week 4

1) Complete and submit Clinical Encounter Form (Word Fill-in). List the 10 most frequently encountered medical problems in your selected community practice. Briefly describe the increase in clinical competency you have acheived in this course.

2) Complete and submit Service Learning Project Report (Word Fill-in).

      Product: You may have negotiated to submit a document or other product useful to your selected community. If so, submit the product to the course director and provide the following information:

• Describe how the product was introduced in the setting
• What insights, skills and knowledge did you gain from developing the product

      Service: You may have negotiated to provide a specific service to your selected community. If your service learning project was a service, you must provide the following:

• Describe the service
• How many people it impacted
• What were the outcomes of the service
• What insights, skills, and knowledge did you gain from providing the service
